This Home Health RN position is based in Jeffersonville, Indiana, a welcoming riverfront community situated just across the Ohio River from Louisville, Kentucky. As a Home Health RN, you will deliver skilled nursing care to patients in their homes throughout Jeffersonville and the surrounding communities. You will provide care for a diverse adult and geriatric population, often managing complex medical conditions such as heart failure, COPD, diabetes, post-surgical recovery, and wound care needs. This role offers a high level of autonomy and meaningful one-on-one time with patients, allowing you to build strong relationships and support patients in maintaining independence in the comfort of their own homes.

Typical responsibilities include completing initial and follow-up assessments, developing and updating individualized care plans, providing direct skilled nursing care, monitoring patients’ clinical status, and educating patients and their families on disease management, medications, and safety in the home. You will collaborate closely with an interdisciplinary team that may include physical, occupational, and speech therapists, social workers, and physicians to coordinate comprehensive, patient-centered care.

A typical day in this role involves managing a caseload of home visits within an assigned territory around Jeffersonville. Depending on acuity and visit type, you can expect multiple patient visits per day, including start-of-care assessments, routine follow-up visits, and discharge visits when appropriate. You will document care and communicate with the broader care team using an electronic medical record system, often via laptop or tablet, supported by office-based coordination staff.

The work schedule generally offers daytime hours with some flexibility to accommodate patient needs, with rotating on-call responsibilities as determined by the employer. Patient visit volumes, travel radius, and specific shift expectations will be clarified by the hiring organization, but the role is designed to support a manageable workload with strong clinical and administrative support.
