Join our team as an RN-Case Manager in the Palm Beaches, Florida. The facility is a Magnet-recognized teaching hospital known for its comprehensive service lines and commitment to patient-centered care. As an RN-Case Manager, you will coordinate patient care, ensure effective discharge planning, and facilitate communication among healthcare providers.
To qualify, you must have a valid RN license, at least 2 years of case management experience, and proficiency in electronic medical records (EMR). Strong organizational and communication skills are essential.
Recommended experience includes familiarity with utilization review and discharge planning processes, as well as experience in a hospital setting.
