Key Responsibilities of the SNF RN:

• Provide high-quality nursing care to residents in a skilled nursing/long-term care setting

• Perform thorough assessments and monitor changes in resident conditions

• Administer medications and treatments according to facility protocols

• Collaborate with physicians, therapists, and interdisciplinary care teams

• Support ADLs, care planning, and family communication

• Maintain accurate and compliant documentation within the EMR system

• Ensure resident comfort, dignity, and safety at all times

Requirements:

• Active West Virginia RN license (or ability to obtain prior to start)

• BLS required

• Minimum 2 years of recent SNF, LTC, or Rehab RN experience

• Strong assessment and communication skills; experience with geriatric care preferred
