Seven Healthcare ;is seeking an experienced and highly organized ;Travel Registered Nurse (RN) – Home Health Case Manager ;for a contract opportunity in ;Pleasant View, TN offering $2,100 weekly. This ;5x8s day shift, ;you will ;Will be covering Montgomery/Cheatham/Robertson County and Sumner area as needed, aswell as a mileage reimbursement of $0.41 per mile.

Key Responsibilities of the Travel Home Health Case Manager:

• Coordinate patient care plans and transitions from admission through discharge

• Perform utilization review and ensure medical necessity for hospital services

• Collaborate with interdisciplinary teams, including social workers and providers

• Educate patients and families on resources, follow-up care, and community support

• Advocate for optimal health outcomes and efficient use of healthcare resources

• Maintain accurate documentation and compliance with regulatory standards

Requirements ;of the Travel Home Health Case Manager:

• Active RN license ;in Tennessee (or compact eligible)

• BLS certification required

• Vaild Drivers licence and Vaild Car Insurance required

• 2+ years of recent experience in case management

• Strong knowledge of discharge planning, insurance authorization, and community resources

• Strong experience in Hospice and HCHB.
