Key Responsibilities of the Travel Case Manager Nurse:
- Coordinate and oversee the patient’s continuum of care across various healthcare settings
- Conduct comprehensive assessments and develop individualized care plans
- Collaborate with physicians, nurses, social workers, and other care team members to ensure seamless care transitions
- Monitor patient progress and modify care plans as needed to achieve clinical and financial goals
- Provide education and support to patients and families regarding diagnoses, treatment options, and community resources
- Facilitate discharge planning, including referrals to home health, rehabilitation, or long-term care facilities
- Maintain compliance with hospital policies, regulatory standards, and documentation requirements
- Participate ;in performance improvement initiatives and case reviews
Requirements ;of the Travel Case Manager Nurse:

Active California Registered Nurse (RN) license

Current BLS certification
- Certified Case Manager (CCM) or Accredited Case Manager (ACM)

Minimum of 2 years acute clinical nursing experience
- Some weekended coverage needed (Mostly on Saturdays)
