Travel Inpatient Care Manager

Key Responsibilities of the Travel Inpatient Care Manager:

• Coordinate and manage patient care across the inpatient continuum to ensure quality and efficiency

• Collaborate with physicians, nurses, social workers, and interdisciplinary teams to develop and implement care plans

• Facilitate discharge planning, ensuring safe and appropriate transitions of care

• Assess patients’ medical, psychosocial, and financial needs to optimize outcomes

• Monitor utilization of hospital resources and promote cost-effective care

• Educate patients and families on post-discharge care, community resources, and follow-up needs

• Ensure compliance with hospital policies, CMS regulations, and accreditation standards

Requirements of the Travel Inpatient Care Manager:

• Active RN license (Massachusetts or compact state) required

• BLS certification required

• Minimum of 3 years of recent inpatient care management including EMR experience. ;

• Strong knowledge of discharge planning, utilization management, and clinical pathways

• Excellent communication, critical thinking, and organizational skills
