Key Responsibilities ;for a Travel Home Heath Physical Therapist:

• Evaluate patients’ physical function and mobility to develop customized care plans

• Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions in home settings

• Educate patients and caregivers on safe mobility techniques and home exercise programs

• Collaborate with physicians, nurses, and other members of the care team to ensure coordinated care

• Monitor and document patients’ progress toward established goals and adjust treatment plans as needed

• Maintain compliance with all documentation, safety, and regulatory requirements

• Participate in interdisciplinary case conferences and discharge planning as appropriate

Requirements for a Travel Home Heath Physical Therapist:

• Valid Physical Therapist license in the state of West Virginia ;
• Doctorate or Master’s degree in Physical Therapy from an accredited program
• Minimum of 1 year of recent experience in a Home Health setting
• BLS certification (AHA or equivalent)
• Strong communication, problem-solving, and patient care skills
