No Dysrhythmia Exam Required Must have 1 year of experienceMainly 4-10 hour days; with Wednesday be off day.OH license in speech language pathologistC’s from ASHAITO Prescreen/Manager follow up and offerUnit Specific DetailsPeds 90%, Adults 5%, Inpatient 5% Very easy bed side swallow testing for inpatient. Caseload is typically 12-14 patients daily Experience with a pediatric caseload including treating disorders in articulation, language delays, childhood apraxia of speech and AAC would be useful in this position.We also need someone with experience in Adult swallowing/language/cognition. (However the majority of this position is outpatient pediatrics there will also be some care required at a small adult inpatient hospital and 2-3 adult outpatientsCommon patient conditions: Autism, cerebral palsy, Down’s Syndrome, early intervention, developmental delay as well as adults with CVA, dementia, acute encephalopathy and pneumoniaJob DescriptionEvaluates, plans, and implements Speech-Language Therapy services for patients according to standards of practice of the profession, laws governing practice by the State of Ohio, and facility and department policy and procedures. Collaborates with the patient, family, and treatment team to maximize functional outcomes for the patient. Obtains and interprets prescriptions for services with physician/referral source as required. Assesses and evaluatespatients using appropriate non-standardized and standardized tests. Ensures completion of patientpre-certification/authorization requirements as needed. Demonstrates knowledge in growth and development necessary forevaluation of adolescent, adult, and geriatric patients (pediatric as required by site). Develops individualized treatment program consistent with patient s age, diagnosis, functional status, and personalgoals. The patient and/or family/caregiver are included in development of attainable goals. Selects and administers appropriate techniques, modalities, and interventions to maximize outcomes. Monitors patient sresponse to treatment and modifies the plan as needed. Demonstrates knowledge in growth and development necessary fortreatment of adolescent, adult, and geriatric patients (pediatric as required by site). Locations for treatment andevaluation include the hospital/clinic and settings in the community. Effectively communicates to patient, family/caregiver and treatment team both orally and in writing. Collaborates with the patient, family/caregiver and treatment team in planning for discharge. Provides patient andfamily/caregiver education and training. Makes appropriate discharge recommendations and obtains and/or recommendsappropriate assistive equipment. Promotes the profession by participating in the Clinical Education program and providing education to others. Maintains competency by participation in continuing education, in-services and other learning opportunities. Meetsstandards for all required reviews of competency. Shares responsibility for common goals of the team by carrying caseload consistent with peers, maintaining the workenvironment and inventories, adjusting individual responsibilities to meet needs of department/work environment andpositively representing department and profession to others. Participates in process improvement by identification of improvement opportunities for patient care, data collection andsolutions to identified problems. MINIMUM EDUCATION Required Minimum Education: Graduate Degree (i.e. Master’s Degree) Specialty/Major: Speech Language Pathology LICENSURE/CERTIFICATIONS (must be non-expired/active unless otherwise stated): Required: State of Ohio license in good standing with a Certificate of Clinical Competence (CCC) from the AmericanSpeech, Language, and Hearing Association or a conditional license under a supervised Professional Experience Planapproved by the Ohio Board. Current CPR certification. Preferred: Lee Silverman Voice Treatment (LSVT) and vital stem training. MINIMUM QUALIFICATIONS Minimum Years and Type of Experience: Experience preferred but not required Other Knowledge, Skills and Abilities Preferred: Experience with electronic medical records Primary Skills: BLS, SLP Job certifications: SLP, Raise the Bar Attestation Job credentials for Professional: BLS
