Specific shift dates will be mutually agreed upon based on facility needs.Call-Off PolicyThe Facility reserves the right to cancel a scheduled shift up to two (2) hours prior to the shift start time without financial penalty.If the Facility cancels within less than two hours, the Clinician will be compensated for two (2) hours of pay at the contracted hourly rate.If the Clinician voluntarily declines a shift or is unavailable, no compensation will be provided.Shift Requirements: Variable 8- and 12-hour shiftsFloat Requirements: May float to Admissions (Acute Psych) if neededOn call/Call Back: NoneWeekends: Must be available to pick up weekend shifts as neededHolidays: May be scheduled for holiday shifts depending on staffing need Required certs to be attached upon submittal: WI RN License and BLS. CPI helpful, but not required.Minimum experience: 2+ years in SNF/LTC. or PsychEMR: Matrix Care – not required. Previous electronic charting, helpful. Unit Information: 2nd Floor LTC/SNF 1 locked unit (16 beds). 1 unlocked unit (12 beds). Average LOS: 40 Days. At least one CNA on at all times. Nursing managers and DON are always available via phone at night.Patient ratios: 1:6-7, ratios for LTC/SNF 1:12 Scrub requirement: Any professional scrubs (NO advertisements listed on them). Dress Code: Hair must be pulled back and of natural tones (no extremely bright or vibrant colors), no hanging jewelry – stud earrings, facial piercing must be small posts (no rings or hoops), no tongue piercings.
