Collaborates and communicates with interdisciplinary teams in all phases of discharge and care planning processes. Communicates with patients, family, medical staff, care team members, students, supervisors and outside contact. Evaluates the process of initiating plans for discharge and/or transitions of care and ongoing case management needs for patients and/or families is required. Supervises staff on the coordination and facilitation of patient care progression throughout the continuum of care. Participates in the interviewing/hiring process by reviewing job profiles, reviewing candidateapplications/resumes, developing behavioral interview questions, conducting and evaluatinginterviews in a timely fashion and providing feedback/recommendations on hiring decisions. Ensures referrals are initiated to appropriate next level of care (e.g. acute care, home health care,hospice, SNF, LTAC, clinics, TCM), in a timely manner, and coordinates ordering of medicalequipment/supplies for safe discharge planning or ongoing care planning. Works with staff to facilitate family meetings with all patients as requested by patient, family orhealthcare teams. Leads collaboration with physician and members of the interdisciplinary teamsto facilitate care for designated case loads. Supports the contact of families, physicians or other healthcare team members in anticipation ofspecials needs of the patients or families, in a timely manner, to facilitate effective treatmentplans. Provides educational materials and methods in support of team members’ abilities to documentappropriately to patients’ level of understanding and motivation. Ensures that patients/families are appropriately assessed for needs with a developed andindividualized continuing care plan in collaboration with physician. Educates staff on variances in patient responses and how to proactively adjust the care plan incoordination with the interdisciplinary team. Supervises staff on the screening process with patients within 24 hours of admission forappropriate services and interventions. Evaluates the team members under their supervision assuring proper knowledge gaps are closedand provided necessary education to develop the case manager to the highest level possible. Participates in evaluation of metrics and problem solves with the case managers under theirsupervision to achieve operational goals and metrics Understands how to data mine individual productivity and evaluate length of stay on theirassigned units with the case managers. Works proactively with the Clinical Directors for collaboration on discharge. Assures staffing is fair and equitable for all units and the facility is covered each day.
