Seven Healthcare is seeking a highly skilled and detail-oriented RN Care Manager to join a reputable healthcare organisation based in White Plains, New York. This role is ideal for an RN with robust clinical experience and a passion for care coordination, patient advocacy, and utilisation management within a dynamic hospital environment.

Key Responsibilities
- Conduct comprehensive assessments and develop individualised care plans
- Coordinate care transitions and collaborate closely with interdisciplinary teams
- Perform utilisation reviews using Milliman/InterQual criteria
- Advocate for patient needs while ensuring appropriate care levels and resource use
- Educate patients and families on care plans, services, and available resources
- Navigate payer requirements and regulatory processes
- Participate in rotating weekend coverage (one weekend per month)
Schedule Details
- Monday–Friday: 9:00 AM – 5:00 PM
- Weekend rotation: one Saturday–Sunday rotation per month
Qualifications
- Bachelor of Science in Nursing (BSN) required
- Active New York State RN licence
- PRI Certification (or must obtain within three months of hire)
- SCREEN certification strongly preferred
- Care Management certification preferred (e.g. CMC, CCM)
- Minimum three years of clinical RN experience (emergency department, critical care, or medical/surgical preferred)
- Previous case management experience preferred
- Knowledge of healthcare financial, regulatory, and payer systems
- Experience with state, local, and federal healthcare programmes
- Proficiency in Milliman/InterQual criteria
- Strong computer and documentation skills
