Clinical Community Bed Manager

Tripod Partners are seeking an experienced Clinical Community Bed Manager to provide expert leadership and support for the Discharge to Assess model across pathways 2 & 3. This role is crucial in managing discharge flow and ensuring patient safety and satisfaction across community beds in Bedfordshire and Luton.

Key Responsibilities:

• Manage discharge flow from Bedford, Luton & Dunstable Hospitals, and neighboring hospitals into community commissioned and spot-purchased beds.
• Provide clinical oversight of patient caseloads, ensuring appropriate and timely decisions.
• Collaborate with system partners, including acute discharge teams, MDT colleagues, and health, mental health, and social care professionals.
• Ensure good flow through community beds by maintaining robust operational processes and building strong relationships with system partners.
• Deliver daily situational reports (sitreps), including updates on bed stock, length of stay, cost, and breach escalations.
• Support the review and development of community bed functions and contribute to ongoing P2/3 transformation work in Bedfordshire and Luton.

Position Details:

• Location: Bedfordshire Community Health Services
• Start Date: Early February 2025
• Duration: 8-12 weeks
• Working Hours and Days: To be confirmed
• Rate of Pay: £37.44 per hour
• Pay Type: Umbrella or PAYE available

Requirements:

• Strong clinical leadership skills and experience in discharge flow management.
• Expertise in working with Discharge to Assess pathways, particularly pathways 2 & 3.
• Ability to build and maintain effective relationships with system partners and operational teams.
• Proficiency in providing daily updates and reporting on bed management metrics.
