DESCRIPTION

  • Join a dedicated Adult Social Care team, delivering person-centred support to adults with care and support needs, promoting independence, dignity, and wellbeing.

  • Undertake statutory assessments and interventions under the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation, ensuring timely and proportionate responses.

  • Manage a varied caseload involving safeguarding enquiries, mental capacity assessments, hospital discharges, and care planning for individuals with complex needs.

  • Work in close partnership with service users, families, carers, and multi-agency professionals to provide holistic and coordinated support.

  • Apply strengths-based, outcome-focused practice, promoting choice and control for adults while supporting least restrictive options and community-based solutions.

  • Benefit from regular supervision, CPD opportunities, and a supportive working environment that values professional development and workforce wellbeing.
