Portsmouth City Council is seeking an experienced Locum Qualified Social Worker (QSW) to join their Adults Social Care Team on a 3-month contract. This role involves working with adults with care and support needs, undertaking assessments, and delivering person-centred interventions in line with the Care Act 2014. The ideal candidate will have experience in adult safeguarding, community-based work, and multidisciplinary collaboration.
Key Responsibilities:

Conduct strengths-based assessments and develop tailored care and support plans.

Carry out safeguarding enquiries in line with Section 42 of the Care Act 2014.

Work collaboratively with health, housing, and voluntary sector partners to coordinate effective support.

Review care plans regularly to ensure they remain appropriate and outcome-focused.

Provide advice and advocacy to individuals and their families, promoting choice and independence.

Maintain accurate, clear case records in compliance with Portsmouth City Council policies and statutory obligations.
