If clinician previously worked with Marshfield, they must have an end date 6 months prior to their submission date.*All clinicians must clock in and out at facility- # of patients treated per day: Max 13 (45 min apt)- General Certification Requirements: BLS, PT License – Common diagnosis / Types of patients: Spine related, Shoulder pain, Total hip & Knee pain, hip, pain/surgery, Balance / fall risk Total hip & Knee Replacement. – Acute Inpatient, Acute Rehab, Skilled, Outpatient?: Outpatient and Inpatient PT – Experience or ‘Must have’ skills & common job duties: Hip & Knee Replacement, Inpatient experience with discharge for hip and knee (big ones) and rounding for inpatient- Rehab Aides: PTA’s: No- Shifts & Scheduling: M F 8-5pm, Will work with manger on inpatient scheduling. – Weekend Requirements: None- Holiday Requirements: None- Scrub Color/Dress Code: No scrub unless want to wear for inpatient Business Causal- Additional Notes: EMR Cerner Cancellation Policy:Client may cancel up to 12 hours every 13 weeks Charting System:Cerner BSN Required:False License to Submit:Yes Trauma Level: Care Setting:Post Acute Division:Allied Shift Notes:Monday – Friday Required Certifications:BLS (AHA)
