Personality Fit: Must be flexible working on a team.Dress Code: Scrub Color: NONE- Black pants and collar shirt Any unacceptable attire: shorts, open toed shoes.Additional Information:Under the direction of a physician order, plans and implements specific physical/occupational therapy evaluation and treatment plan programs. The treatment is based on the patient s physical, developmental and psychosocial needs. Provides advisory service to other team members to provide quality patient care. Functions under the direct supervision of the Director and Assistant Director. Performs related duties as assigned.Responsibilities Performs Evaluation including the following components as indicated by patient need: 1.1 To include Orders, Diagnoses, History, 1.2 Subjective Finding to include but not limited to: Patient Report of Pain, Patient/Family Goals, Prior Level of Function, Mental Status, Discharge Plan 1.3 Motor and Sensory Function to include but not limited to: Range of Motion, Muscle Tone, Strength, Coordination, Balance, Fall Risk and Sensation and Edema 1.4 Functional Mobility / Transfers including Home/Work/School environment 1.5 Gait / Wheelchair Mobility to include but not limited to: Distance, Device, Assistance, Deviation 1.6 Endurance including as indicated: Duration of Activity, use of Supplemental Oxygen, monitoring Vital Signs 1.7 Plan and Recommendations to include: Patient-centered, Function Focused, Measurable Goals, Patient/Family Education, Equipment Needs and Follow-up Services Provides Treatment to include the following components: 2.1 Interventions to meet Functional, Psychosocial, Cultural, Educational and Spiritual needs. 2.2 Therapeutic modalities may include but not limited to Ultrasound, Hot Packs, Electrical Stimulation,Whirlpool, Transdermal Medication 2.3 Therapeutic interventions to include but not limited to Therapeutic Procedures, Neuromuscular Reeducation, Gait Training, Manual Therapy, Therapeutic Activities, Wheelchair Management, Wound Care, Orthotic Management, Prosthetic Management 2.4 Demonstrates effective interdisciplinary collaboration and documentation of said collaboration Discharge Planning 3.1 Education provided to Patient/Significant Other/Caregiver to include but not limited to the Home Program, Equipment and Instruction in the use of Equipment 3.2 Recommend Follow-up Care, if any, which may include Home Program, Home Health Care, Skilled Nursing, Inpatient Rehabilitation, or Outpatient Therapy Documentation 4.1 Completes all documentation in a timely and thorough manner in accordance with regulatory guidelines Productivity Supports and implements budgetary practices in the planning, organization and efficient execution of work activities. 5.1 Meets established departmental productivity standards, conserves resources, and recognizes the value of cost containment while maintaining quality standards. Physical demands are attached plus push at 40 lbs., pull 60 lbs., and grip up to 50 lbs., pinch up to 15 lbs., kneel, crouch, and crawl minimally. Equipment Used: May occasionally work with office equipment; i.e. copy machines, telephones, computer.
