Data Collection: Unit Information: Location: SDMC Building: Rehab Hospital as well as Acute Hospital Floor: IPR, OP, Acute Order Detail: Facility Department Number: 76279 PT; 76679 – OT Discipline: PT and OT Quantity: 2 PT, 2 OT Target Start Date: ASAP Shift: 7/10 hrs weekdays/weekends Call Requirements Weekends: weekend rotation pending utilization during the week, 1-2 weekend days/4 weeks General Information Tell us about the unit(s): Number of Beds 64IPR; 400 bed acute, full OP services, neuro experience required Number of Staff 160 Type of staff: o Day Shift (example 1 charge, 3 RN, 2 CNA, and 1-3 sitters) o Night Shift – Patient Ratios 75% direct patient care/8 hr shift Type of equipment – EMR Permanent Positions on Unit Avaliable? (Y/N) 2 PT vacancies in IPR, 1 PT vacancy in OP; 1 PT vacancy in acute Typical hiring profile Skill Set Most recruited for: (Travel RNs, local CNAs and sitters) Must have: candidates without these skills will not be considered for the role. Neuro background SCI, BI, in addition to stroke rehab Experience in multiple settings Acute, IPR, OP settings Preferred or nice to have: candidates with these skills will be considered first. Ability to transition between multiple programs on any given day Understanding of IPR post-acute guidelines; utilization of CARE tool assessment List typical procedures performed on unit(s): Follow schedule provided, coordinate with interdisciplinary team, demonstrate ability to meet productivity standard. Good time management Best personality Fit: Organized, friendly and able to engage with staff, team oriented, effective with communication and collaboration.
