Client rates reduced by $8.00/hour.********Must have the WI state license, or have already begun the process and it is close to being issued – provide target issuance date at time of submission. *******************************Must go through credentialling with this client. The committee meets every 3rd Monday of the month, so if not done with all onboarding 3 business days prior to committee meeting, may have start date pushed out 30 days. **************- # of patients treated per day: 9 to 12 daily.- Common diagnosis / Types of patients: General outpatient orthopedics, Prefer: Pediatrics or Hand Therapy, 2 are peds only, and 1 hand therapist, 1 neuro or lymphedema.- Acute Inpatient, Acute Rehab, Skilled, Outpatient?: Outpatient only.- What areas are they required to cover?: Marshfield Center.- Experience or ‘Must have’ skills & common job duties: Evaluation, treatment planning, re-evaluation, manual therapy. OK with 1 year including internships. Ok first time travelers. Ok with duos. Nice to have Dry Needling, Pediatrics, Certified Hand Therapy, Neuro, lymphedema.- OT Manager/Supervisor:: Aaron Homolka- Rehab Aides: COTA’s:: 3 Appointment Coordinators/Rehab Aides. All positions have appointment coordinator/ rehab aid, to coordinate front end to back end of services, registrations phones, appointing and reappointing, change over rooms, clean and disinfect equipment etc. – Shifts & Scheduling: Monday- Friday: 8am-5pm (8 hour shifts) (some flexibility in hours to get 40 hours/ week).- Weekend Requirements: No.- Holiday Requirements: No.- Scrub Color/Dress Code: Business Casual.- Department specific orientation: 2 days onboarding/orientation: manager meets with them on first day, a lot of CBTs and done by end of first week, and follow to shadow with a therapist and workflow.*MUST note in submission if clinician has ever worked for Marshfield.
