Job Title: Occupational Therapist or Physiotherapist – Band 6

Job details: Occupational Therapist or Physiotherapist required to start ASAP for Community Therapy Wards.

•  
• Provide therapy input within community-based hospital wards
   

• Undertake mobility assessments and develop individualised exercise programmes
   

• Support rehabilitation and reablement of patients
   

• Contribute to effective discharge planning to support safe transitions from hospital
   

• Provide and prescribe appropriate equipment, including manual handling equipment
   

• Supervise and support junior members of staff

Skills & experience required:

•  
• HCPC registered Occupational Therapist or Physiotherapist
   

• Experience working on hospital wards within a rehabilitation or reablement setting
   

• Strong skills in mobility assessment, exercise prescription and discharge planning
   

• Manual handling experience essential
   

• Knowledge of common conditions including arthritis, heart disease, diabetes, dementia, MSK conditions, COPD, orthopaedics and falls
   

• Car driver essential – role requires transporting items of equipment
