Occupational Therapist required for a Double Handed Review Project within a proactive Reablement Team.
This is a newly developed role offering the opportunity to shape and embed Occupational Therapy practice
within the Reablement Service. The post holder will focus on moving and handling assessments and
recommendations to ensure right-sized care, promoting dignity, independence and positive outcomes.

•  
• Complete moving and handling assessments and make recommendations to review and reduce double-handed care where appropriate
   

• Work proactively within the Reablement Team to promote independence and prevent, reduce or delay the need for ongoing services
   

• Work collaboratively with Social Workers, Occupational Therapists, Social Care Assessors and OT Assessors
   

• Provide information, advice and guidance to service users to support wellbeing and independence
   

• Ensure the most appropriate care, equipment, technology and techniques are in place
   

• Support seamless transitions to other teams when required
   

• Maintain accurate records using relevant IT systems (training provided)

Skills & experience: Experienced practitioner with strong moving and handling knowledge,
excellent communication skills, good organisation, and solid computer literacy.

Working hours: 37 hours per week
