MDS Manager

Seven Healthcare is seeking an experienced and detail-oriented MDS Nursing Manager for a full-time, permanent opportunity in Watertown, New York. Key Responsibilities

As the MDS Nursing Manager, you will:

• Oversee and coordinate the MDS assessment and care planning process

• Ensure accurate and timely completion of OBRA and PPS assessments

• Collaborate with interdisciplinary team members to ensure appropriate care planning and documentation

• Monitor and analyze MDS data for accuracy and regulatory compliance

• Train and support nursing staff on MDS processes and changes in regulations

• Participate in quality improvement initiatives, audits, and survey preparedness

• Serve as a liaison with CMS and state agencies regarding MDS-related requirements

Qualifications

• Active RN license in the state of New York

• Minimum of 2 years of MDS experience in a skilled nursing or long-term care setting

• Knowledge of Medicare/Medicaid regulations, RAI process, and PDPM

• Strong organizational, analytical, and leadership skills

• Experience with electronic health records (EHR) and MDS software (e.g., PointClickCare) preferred

• RAC-CT certification a plus (or willingness to obtain)
