Licensed Practical Nurse (SNF)

Seven Healthcare is seeking a dedicated and experienced Local ;Licensed Practical Nurse (LPN) for an evening ;shift position at a Skilled Nursing Facility (SNF) in Seaford, Delaware offering a weekly rate of $1,350. Key Responsibilities:

• Provide compassionate, hands-on care to residents in a skilled nursing facility

• Administer medications and treatments in accordance with care plans and physician orders

• Monitor patient conditions and report changes to RNs or physicians

• Assist with wound care, documentation, and ADLs (activities of daily living)

• Collaborate with CNAs, RNs, and interdisciplinary teams to support resident care goals

• Ensure compliance with facility policies and state/federal regulations

• Maintain accurate and timely clinical records

Requirements:

• Active LPN license in the state of Delaware (required)

• Current BLS certification (required)

• Minimum 1 year of LPN experience in a long-term care or SNF setting (preferred)

• Strong communication, clinical, and organizational skills

• Ability to work independently during night shifts while being part of a team-oriented care model
