Case Manager

Seven Healthcare is seeking a compassionate and detail-oriented Case Manager RN to join a collaborative care team in Portsmouth, New Hampshire. This full-time contract offers consistent day shifts, excellent weekly pay, and the opportunity to support patients through care coordination and discharge planning.

Responsibilities

• Coordinate and oversee patient care across the continuum

• Develop and implement individualized discharge and care plans

• Collaborate with providers, families, and interdisciplinary team members

• Ensure quality outcomes and cost-effective care delivery

• Advocate for patient needs and resources

Requirements

• Active New Hampshire RN license (or ability to obtain)

• Minimum 2 years of case management or acute care experience preferred

• Current BLS certification required

• Strong assessment, communication, and organizational skills
