Pay Rate: $2105.00 – $2319.80

Required Qualifications
Home Health and Oasis experience
Evergreen’s minimum booking requirement is 25 days out
Evergreen is requiring all employees and contract staff to have the Covid and Infuenza vaccine.
Per communication with all employees, there are only 2 options to opt out:
“Those of our colleagues who decline the vaccine for medical or religious reasons will not be subject to criticism or penalized in any way. We WILL require the completion of a formal declination form and confirmation in writing from the individual’s physician or religious leader. Personal preference declinations will not be accepted.”
Client will not allow a negative titer result if there is any known history of past positive TB results (will need to get CXR)
